In Car Video Request

To request an in car video please complete the following:

1. Fill out and print the request for video form. 
2. Submit completed form along with $40.00 per request, check or money order made payable to the City of Independence.
3. Mail or walk in request with completed form and payment.
4. Allow 48 hours for the request to be processed.
5. You will be notified via phone when the request has been completed. 
6. When picking up the video, you will need to come in person as you have to sign a form for release. 
7. If there is no video available, you will be notified by mail and your check or money order will be returned.
