Please read the following instructions before pre-registering for an H1N1 clinic.

• Patients are to register themselves and their children; spouses must register themselves.
  
  
• Registrants will be electrically signing off on the consent for themselves and their dependents.
  
  
• Dependents 18 years or older must register themselves and consent for vaccination themselves.
  
  
• It is strongly recommended that participants print the 'fast track pass' for each individual that is registered and consented for vaccination (see the final screen after each individual's consent).
  
  
• Registrants must note the email and password or username and password configuration that they create during registration. This will provide the registrant access back into the solution if they need to finish consenting themselves or their dependents or if they need to reprint each participants 'fast track pass.'
  
  
• Participants receiving two vaccinations that have previously registered within the solution for their first vaccination must create a unique account for the second vaccination. This requires that a different email or new username be entered for this second registration.
